City of Farmington 1095-C Delivery Changes 

Pursuant to federal Affordable Care Act (ACA) reporting requirements and recent changes in distribution rules, employers are no longer required to mail forms to all employees and participants. Instead, employers may provide Form 1095-C only if requested.

Your Right to Request a Statement

Individuals who otherwise would receive Form 1095-C for the 2025 calendar year may request a copy of their form from the City of Farmington. This notice will be posted on the city website at www.farmgov.com under the Clerk’s Office Section.

Additional Notes

  • This change will NOT affect the distribution of your Form W-2: Wage and Tax Statement.
  • Not everyone will receive a Form 1095-C, including those who were not in a full-time position and were not offered medical coverage.
