Health Needs Assessment Survey

An opportunity for residents of Farmington / Farmington Hills to become part of a Health Needs Survey for your own community!
The Farmington / Farmington Hills Health Advisory Council is conducting a Community Health Needs Assessment Survey. The Council is a diverse group of members from the school system, faith-based groups, local employers, city representatives, among others. The survey is being conducted as part of the Accountable Care Act to focus on health. The information collected will help us plan how to work best together to support the health of the residents of Farmington and Farmington Hills.
Participating in the survey is completely voluntary and your survey answers will be anonymous. There will be no way to identify anyone who participates in the survey. All survey responses will go directly to Public Sector Consultants, who will be collecting and compiling the data. The survey is open now through July 22, 2013. All survey responses are due on or before July 22, 2013. The survey should only take 5-10 minutes of your time.

If you prefer a printed scantron survey, you can call Botsford Hospital Community Relations at 248-442-7986 to have a printed copy of the survey mailed to you.
Please keep in mind you must complete the survey by July 22 for your answers to be included in the final results.
